Abstract
A 37-years-old Indian female presented with left eye severe proptosis for one-week associated with diplopia and progressive diminution of vision. Patient underwent sinus surgery a year ago. CT scan revealed a mass infiltrating left orbit and arising from the maxillary sinus. MRI showed sinonasal polyposis, collection in paranasal sinuses with bony erosion and extension into bilateral orbits. Otorhinolaryngology consultation was sought and in view of vision threatening proptosis –with likely orbital compartment syndrome – emergent endoscopic guided exploration and orbital decompression was undertaken. Rapid improvement in the form of resolution of proptosis, normal extraocular movements and visual acuity. Cytology and microbiology ruled out infectious or malignant etiology and helped in early definitive diagnosis. Histopathological evaluation confirmed mucopyocele. Patient was stable during follow up of one year.
