Intraoperative complications are inevitable in cataract surgery. Inadequate capsular bag support is a nightmare for all cataract surgeons. There are multiple techniques for intraocular lens (IOL) implantation following inadequate capsular bag support like anterior chamber IOL, iris-sutured posterior chamber IOL and scleral sutured posterior chamber IOL. Immediate access to vitreo-retinal surgeon is not possible in all institutes. It is crucial to choose the appropriate technique for better surgical outcome. The technique of single haptic iris fixation of posterior chamber IOL is challenging but a very attainable skill for anterior segment ophthalmic surgeons. This technique shortens the operating time, reduce further complications like expulsive hemorrhage and gives good IOL centration. A well centered IOL near nodal point and rotational axis of the eye is a key to overall success of cataract surgery.
