Abstract
The video will showcase how phacoemulsification can be successfully completed in a posterior polar cataract even in the presence of a pre-existing posterior capsule rupture if guarded steps are followed. Two cases will be discussed, first being a soft cataract in a young individual and second a medium-hard cataract in an old individual. Following the creation of the main incision, an oval rhexis is fashioned in both cases. No hydrodissection is done. Rather, in the first case, layer-by-layer hydro-delineation is done. In the second case, the nucleus is divided after sculpting without wide separation following which cross chopping done. The nucleus is emulsified with no rotation in both cases. Cortex aspiration is done by the tangential stripping method. Posterior capsulorhexis is fashioned. Multi-piece IOL is implanted in the sulcus with optic capture completing the cases uneventfully. This resulted in the acceptable anatomical and visual outcomes.
