Abstract
PXF syndrome is age related genetically determined(LOX L-1),environment related multifactorial systemic disease having deposition of fibrillogranular amyloid over endothelia, iris,capsule,TM&lens zonules.Previously known as cold climate disease but no race/region is free.It poses various threats like zonular weakness, poor pupillary dilatation, unstable/decentered PCIOL,vitreous loss, increased post op inflammation, corneal decompensation&secondary open angle glaucoma.We had shown how to tackle all complex situation while doing phaco in PXF.Poor pupillary dilatation can be managed by Iris hooks/pupil expander rings.Zonular dialysis can be managed by CTR/Cione.Capsulorhexis must be of ample size using OVD&sharp instruments.Hydrodissection-gentle&multiple.Chopping-zonular friendly direct chop under low phaco parameters.Tangential stripping in cortical cleaning.PCIOL-Hydrophobic,heparin coated.So by taking proper pre, intra&post op measures-successful phaco is definitely achievable.
