Abstract
The presence of dark spots following femtosecond laser application in RELEX SMILE suggests incomplete dissection of the lenticule. This video demonstrates a torn lenticule with extension of the side cut in a case, where the surgeon has used a combination of techniques like sharp dissection (where a sharp dissector was used to dissect out the area of resistance in a meticulous manner) and lenticulo-rhexis (like a continuous curvilinear capsulorrhexis in cataract surgery) to manage this difficult complication. At the end of the procedure, both the pieces of the removed torn lenticule, apposed liked a perfect fit, indicating the absence of any lenticular remnants in the interface. The flap was well apposed with the patient having an uncorrected visual acuity of 20/60, 20/30 and 20/20 on post-operative days 1, 7 and 21.
