Ocular tick infestation is a rare occurrence. We report a 61 year old male with complaints of watering, pain and discharge in left eye for two days. On examination, there was left upper eyelid oedema with erythema and live parasite on lid margin. Patient was treated with tablet Ivermectin 12mg stat and Vaseline jelly applied over parasite. After four hours, parasite was found dead which was removed along with its pincers under topical anaesthesia through slit lamp. The parasite was identified it as Rhipicephalus haemaphysaloides, a hard tick which is an obligate ectoparasite of domestic cattle and dogs. Human infestations of this tick found to be associated with diseases Kaysanur Forest Disease and Crimean-Congo hemorrhagic fever. Patient was treated with appropriate antibiotic local eye application and also found negative with these associated diseases.
