Abstract
A 4 months infant with failure to thrive was brought with 4 days history of redness with white discoloration in left eye. On repeated questioning mother admitted instillation of breast milk to treat the discharge. Baby was born preterm and had undergone colostomy for imperforate anus. Examination of left eye revealed lid edema, congestion with yellowish purulent discharge. Large ulcer with total corneal thinning with central desmatocele and thick exudates at base and anterior chamber were noted. Right eye was normal. Gram staining revealed gram negative bacilli. Infant was treated with IV ceftriaxone and nutrition support. Topically tobramycin and homide eyedrops and chloromycetin eye ointment was instilled. Culture report was positive for pseudomonas. Infection was controlled over two weeks and patient is planned for keratoplasty. Pseudomonas infection should always be suspected in cases with rapid worsening as prompt treatment can prevent vision and life threatening complications.
