Abstract
Case report: This is a case report of management of acute hydrops by DALK as a primary line of treatment. The decision of DALK was taken after serial preoperative evaluation with ASOCT and incorporating these inputs intraoperatively to prevent accidental perforation.A 21 year old Female patient, known case of Both eyes Keratoconus presented to our cornea clinic, with complaints of sudden diminution of vision in left eye since one week.Her best corrected visual acuity was 6/9 in Right Eye and 1/60 in left eye. She was diagnosed as Left eye Acute Hydrops. After performing initial work up including patchymetry,NCT,k-reading, and ASOCT,DALK was done in LE.conclusion: Patient improved from UCVA of 1/60 to immediate post operative UCVA of 4/60.Postoperative day 1 , ASOCT showed no interface cleft or double AC.1month post operative vision improved to 6/60.
