Abstract

AIM:Description of rare case of Purtscher-like- Retinopathy in Pre-Eclampsia with IUD.DESIGN:Observational case report.METHOD:Clinical Examination and investigation of b/l blindness in pre-eclampsia with IUD.RESULT:We report a case of Purtscher like retinopathy leading to b/l blindness in 27yr old pre-eclamptic pt with sudden painless profound vision loss after worsening of pre-eclampsia f/b IUD. Fundus examination showed multiple white retinal exudative patches around the disc & macula.FFA showed hypofluorescence at the macula & macular ischemia.OCT showed inner hyperreflectivity s/o Ischaemia with outer hyporeflectivity s/o retinal edema.PST Triamcinolone acetate showed poor visual recovery.CONCLUSION:Pre-eclampsia with unknown factor probably led to compromised optic nerve microvasculature autoregulation further aggravated by IUD resulting in Purtscher-like-Retinopathy. No effective treatment is known. Partial recovery reported with high doses of Intravitreal steroids.
