Abstract

51yr female dengue pt developed sudden loss of vision, redness & pain in the LE on day 5. 6/36 vision, IOP 46mmHg in the LE with 5mm pupil. RE 6/6 vision, IOP 14mm Hg. Gonioscopy and Anterior segment OCT confirmed AACA. Dengu demands plenty of fluids& we need to give Mannitol.With intensive intermittent Ocular massage IOP was reduced to14 mm of Hg and vision 6/6p in 40 minutes. PI was deferred in view of low platelets (16000/µl). Patient was kept on pilocar & steroid drops.

2 days later she developed AACA in BE, IOP>40mmHg &vision CFCF and corneal haze. With Mannitol & additional topical medications IOP became 8mm of Hg and vision 6/6p in BE. Planned for PI on a later date with acceptable platelets.

The pathogenesis of ocular complications in dengue is not fully understood thus dilemma in management. Literature suggested ciliary effusion with pushing of lens iris diaphragm forward. UBM will be helpful in diagnosis. Then cycloplegics instead of pilocar will be the treatment.
