FP2018 : Papilledema –an atypical presentation of Neurocysticercosis

Abstract
We report a rare case of a man, aged 21 who presented with severe headache & small nodules on both upper eyelids for 1 month. His visual acuity was 6/6 in both eyes with normal anterior segment. Fundus showed bilateral disc edema. MRI brain showed multiple small well defined subcentimetric lesions scattered throughout bilateral cortex,hind brain,tongue ,extraocular muscle, cervical soft tissue- suggestive of Neurocysticercosis (NCC)-Vesicular stage. After neurology consultation,he was started on steroid, antiepileptic & acetazolamide. Few days later he presented with altered sensorium and nausea for which he was started on i.v steroids and antihelminthics. In NCC, parasite locates in brain parenchyma, subarachnoid space with several pathological changes showing clinical pleomorphism of the disease. This case highlights the importance of evaluating patients with clinical presentation of subcutaneous nodules and papilledema for cysticercosis.
