Abstract
To examine a 10 year old male child with sudden painless loss of vision in right eye since 3 days and evaluate role of pulsed IV methylprednisolone in no PL, no RAPD, no fundus abnormalities and no MRI findings.
METHODS
Visual acuity assessment showed no PL in right eye and 6/6 in left eye. Anterior and posterior segment examination was within normal limits.Tests for malingering were negative.CSF analysis and MRI brain was normal.
VEP showed increased p100 latency in right eye.
RESULT
A presumptive diagnosis of retrobulbar neuritis was made and the patient was started on IV methylprednisolone 30mg/kg/day for 5 days.On day 2 of treatment vision in right eye improved from no PL to 6/9 (unaided).There has been no recurrent attack or other eye involvement in 1 month follow up.
CONCLUSION
this was a rare presentation of painless sudden loss of vision with no RAPD and MRI changes with improvement in the visual outcome by IV methylprednisolone.
