Abstract
A 35 year old female presented with painful decrease in vision and inability to move the left eye (LE) since one year. Best corrected visual acuity (BCVA) in the LE was finger counting 1 meter with accurate projection of rays. Examination showed hypotropia, conjunctival chemosis and marked limitation of extraocular movements in all gazes. Computed Tomography (CT) of the orbit showed a mass lesion measuring 22 mm x 14.5 mm in lateral half of posterior orbit with proptosis. Incisional biopsy revealed inflammatory infiltration. Tablet Prednisolone (1mg/kg body weight) was given for four weeks. After reduction in inflammation, a foreign body could now be palpated. Surgical exploration revealed a large wooden foreign body measuring 3.3x1x0.3cm. Postoperatively, drastic improvement in BCVA was noted but eye remained hypotropic. Though intraorbital foreign bodies are not rare, ambiguous history, delayed presentation and non specific CT findings made this case diagnostically challenging.
