Abstract
Trabeculectomy has undergone revolutionary changes since its inception. Judicious augmentation with antimetabolites, selective use of porcine collagen and various modifications of the standard procedure have ensured enhanced success with increased safety. Intensive post-operative follow up and a low threshold for early intervention has resulted in higher proportions of eyes achieving target IOP.
This IC will offer basic guideline of the procedure starting with case selection, safe handling of MMC, ologen in selective cases and use of releasable sutures. It will also discuss the best ways of bleb manipulations including laser suturolysis, staged suture release, ab- interno revisions,conventional needling and repeat filtration when required. Recently sub tenon injection of MMC is emerging as a superior method of delivering the antimetabolite. We will also discuss several surgical benefits of the injection method that translate to potentially better outcome for the patients.
