Abstract
Life of a novice surgeon in private practice is not easy. Everybody expects a good visual outcome even in the worst-case scenario. Doing a phaco in very hard cataract or in advanced hypermature cataract is a nightmare even for a seasoned surgeon. The answer is manual small incision cataract surgery (MSICS). In this video we demonstrated MSICS in a rock-hard black cataract and in an advanced sclerotic hypermature cataract with very good visual outcomes. In the absence of MSICS, result of Phaco in these cases may be disastrous in hands of small timers like me.
