Abstract
Despite technological advancements in cataract surgery, posterior capsular rent (PCR) still occurs. PCR occurs both in the hands of experienced surgeons and neophytes, although with a higher frequency in the latter group. Hedonic adaptation means, ending up where one actually starts, due to cognitive distortion. Predominantly, young surgeons are caught up in this vicious cognitive cycle of PCR during early days of phaco-emulsification, hindering their growth. This video theatre highlights series of non-conventional PCR videos (with proper management). They are compiled in a domino effect screenplay with motion graphics, involving intra-operative decision making from a beginner’s point of view. A Cliff Hanger Technique of IOL insertion has also been proposed for neophytes, when their back is against the wall. At the end, the viewers will be equipped with tools, that can help overcome the limiting effects of hedonic adaptation of PCR, thus creating a greater level of operative awareness.
