Abstract
In this video we describe a different technique for IOL (intraocular lens) implantation without anterior vitrectomy for simple and complex paediatric lenticular conditions.Three children underwent lens matter aspiration with optic capture without anterior vitrectomy for paediatric cataract, posterior lenticonus and persistent foetal vasculature (PFV) with cataract. In each of these cases a modification of the technique was required intraoperatively in order to have a reasonably large posterior capsulotomy for the optic of the intraocular lens to prolapse through it. We had to be extremely careful with the thin posterior capsule in lenticonus and had to use vitreous cutter in PFV. In all these patients no anterior vitrectomy was performed.The art and utility of posterior capture(lock it) in difficult cases of pediatric cataract is highlighted in this movie which is evident by the lack of capsular proliferation(seal it) onto to the visual axis while preserving the vitreous(save it).
