Abstract

Small pupil, a commonly encountered challenging situation for cataract surgeon, can lead to prolonged surgery and a number of complications. Small pupil often leads to a small rhexis and reduced fundal glow. Complications include iris trauma, posterior capsular rupture, zonular dialysis and increased postoperative inflammation.

This video shows small incision cataract surgery in small pupil using iris hooks. After peritomy, scleral incision was given and sclero-corneal tunnel created. Three side ports were made at 3, 12 and 9 o’clock respectively. Dye was injected, viscoelastic given and wound was extended. One iris hook was inserted through the main wound and three from each side ports. After capsulorrhexis, nucleus was prolapsed into the anterior chamber with dialer. Iris hook from the main wound was removed and nucleus delivery was done. Cortex removal was done and IOL was placed in bag. In the end iris hooks from all the three side ports were removed and anterior chamber was formed
