Abstract
Settings: This case was done in a tertiary eye care hospital in eastern India.
Methods: During a mature cataract phacoemulsification capsulorhexis is extended to the periphery.Reverse rhexis was done. Phacoemulsification was completed gently maintaining anterior chamber depth.During cortical cleaning posterior capsular rent occurred.Anterior vitrectomy was done through pars plana.During insertion of single piece foldable IOL it accidentally unfolded in a reverse manner. It is then manipulated very gently to get the right orientation and introduced into the bag.
Results: Patient gained BCVA 6/6 after 1 week follow up.IOL is stable,center and inside the bag.
Conclusion: A simple case sometimes gives rise to nightmarish complications.We should stay calm and act logically to achieve maximum possible outcome.
