Abstract
•After appropriate preoperative investigations including vision, intraocular pressure, refraction, slit lamp examination, and fundus, optical biometry with Barretts calculator was used
•A customised toric IOL(CTIOL) was placed in the bag after an uneventful surgery
•Preoperative toricity- 1.77 at 176 deg
•Postoperative 1 week- refractive surprise of -3.00 degrees cylinder at 90 deg
•Operation theater log- no flipping, no switching of IOL or no wrong IOL power calculation. Detailed investigation shows IOL in place no other external ocular reason
•Explantation was performed and a non CTIOL was implanted. The explanted IOL was sent to the manufacturer who confirmed the wrong toricity of the IOL
Conclusion
•Common causes of postoperative refractive surprise- switching of intraocular lens(IOL), wrong IOL power calculation or flipping of IOL while inserting.
•Rare cause- manufacturing defects to be kept in mind
