Abstract
Synopsis: A 32-year-old male with ocular comorbidities of RP and myopia underwent a phacoemulsification procedure with single-piece IOL implanted in the bag 4 yrs back. He presented with profound vision loss. Had a grossly contracted fibrous bag with a slit-like anterior capsular opening & was dragged superiorly under iris, along with crushed PCIOL uprooted from the zonular support and three-quarters of which was displaced into the anterior chamber in a unique manner. A surgical attempt to open the bag and re-fixate the same IOL failed. Left with option of explantation of the crushed & uprooted IOL bag complex along with intrascleral haptic fixation of a newer IOL to regain centration and useful vision. A description of the microscopic features of the contracted bag and the histopathological findings of the bag would be highlighted in addition to surgical points of interest
