FP2075 : A RARE CASE OF UNILATERAL HYPERTENSIVE RETINOPATHY

Abstract
Background- Hypertensive retinopathy as a result of systemic hypertension is usually bilateral. Unilateral HR is caused by significant stenosis of the internal carotid artery proximal to the origin of the ophthalmic artery on the unaffected side typically at carotid bifurcation due to atheroma & less commonly due to coarctation and arteritis. Case details-A 65yr old male presented with c/o DOV in both eyes since 6 months. Visual acuity: OD-6/9 OS-6/12. IOP (by NCT): OD-17 OS-12 mmhg. Fundoscopy: OD-WNL OS-grade 3 hypertensive retinopathy. His average BP:210/130 mmhg. Carotid doppler: Atheromatous plaque in Rt carotid bulb causing 20-25% luminal narrowing and in Rt internal carotid artery causing 40-45 % luminal narrowing. Atheromatous plaque in Lt internal carotid artery causing 30-35% of luminal narrowing. Conclusion-Unilateral HR is very rare and associated with carotid artery stenosis. Early diagnosis and proper systemic examination can help in preventing deadly outcomes.
