Abstract

Aim: To report a case of serpiginoid choroiditis caused by Mycobacterium mageritense post hysterectomy. Observation: 42-year-old female presented 2 months following laparoscopic hysterectomy with right eye gradual, painless diminution of vision since 6 weeks. Examination revealed right eye BCVA 2/60, vitreous cells and irregular multifocal subretinal lesions involving posterior pole. Fundus Autofluorescence showed speckled hyperautofluorescence with fuzzy (active areas) and sharp (healed areas) margins. OCT showed patchy sub-RPE infiltrates and IS-OS disruption. OCT Angiography revealed flow voids and reduced vessel density. Left eye was normal. MALTIDOF Mass Spectrometry of blood cultures confirmed Mycobacterium mageritense.

Conclusion: Atypical mycobacterial infections resulting from contamination of laparoscopic instruments can rarely lead to debilitating systemic bacteraemia and profound visual impairment, highlighting the importance of aseptic precautions during major surgeries.
