Abstract
A 20 years female presented with sudden rashes all over the body and mucosal ulcerations after Tablet.AMOXYCILLIN+CLAVULANIC ACID for sore throat.A diagnosis of Toxic Epidermal Necrolysis was made. Extensive sloughing of periorbital skin with diffuse conjunctival congestion and membranes were seen.Cornea was hazy with no epithelial defect. Lubricants,cycloplegics,antibiotics started along with systemic management. Recurrent symblepharon occurred on subsequent days and was broken manually.Bandage contact lens (BCL) was implanted in both eyes.As the patient worsened,topical Cyclosporine eyedrops 0.05% was started and there was drastic reduction of ocular signs observed at the end of one month follow up.Vision improved to 6/6.Cyclosporine reduces ocular inflammation by reducing the cytokines and heal the damaged ocular surface.This case highlights removal of offending agent,systemic stabilization,reduction of inflammation with Cyclosporine and promoting ocular surface healing by BCL.
