Abstract
Assessment of patient’s ability to use both eyes together is integral part of comprehensive eye examination. Bagolini test is the gold standard but time consuming, difficult to perform, interpret and lacks availability in many hospitals. Worth 4 dot test’s (W4DT) worth is questioned by many. Aim: To study the worth of W4DT, 20PD and 4PD baseout tests for the assessment of fusion. Methods: In this prospective double blind study, an optometrist performed the Bagolini test and W4DT and pediatric ophthalmologist performed prism tests. Results: Of 55 patients, 26 were males. Mean age was 9.7 years. 5 had squint (2 eso, 1 exo, 2 intermittent exo). 20PD test had the best accuracy [71%, (95% CI, 61%-80%)] followed by W4DT (63%) and 4PDT (59%). The negative predictive value of both 20PD and W4DT was 99%. The specificity of 20PDT was 67% and W4DT was 57%. Conclusion: 20PD test is simple and objective test that can replace W4DT to assess fusion, especially for young and uncooperative patients.
