Abstract

A 6 year male presented with progressive protrusion of R/E for 2 weeks.BCVA right eye counting fingers. Examination revealed chemosed conjunctiva,pupil reacting sluggishly,abaxial proptosis of 32mm on Leuddes with inferior dystopia.Patient was being treated on the line of orbital cellulitis.CT showed heterogeneous soft tissue mass in supero lateral aspect of orbit.A week later MRI showed poorly defined mass extending into superior orbital fissure causing erosion of superior orbital plate of right frontal bone.INCISIONAL Biopsy confirmed diagnosis as Embryonal type of Rhabdomyosarcoma.Patient left against medical advice.Week later presented with fungating mass with severe pain & bleeding.

Patient was started on radiotherapy & chemotherapy with VIE regime.

He is responding well by date.

This case scenario emphasizes the importance of having a suspicion of malignancy on every child presenting with proptosis & also increasing awareness among parents to avoid delayed diagnosis & treatment.
