Abstract
A 23 year old female presented with sudden onset diplopia in right eye with dull boring pain since 5 days. On examination visual acuity in both eyes was 6/6. Anterior segment examination was normal in both eyes with normal pupillary response except for right eye ptosis. Ocular movements of right eye showed no primary and secondary movements but normal movements in left eye. Fundus examination revealed normal findings in both eyes. Systemic examination was also within normal limits. Blood investigations and MRI Brain with orbit revealed no abnormality. Patient was started on systemic steroid after ruling out infective etiology. After 5 days patient came with complaints of swelling all over body. she was started on tablet lasix after physician consultation. Patient was followed up meticulously and improvement in ocular movements and ptosis was noticed. NSOI has variable presentation and is diagnosis of exclusion. We need to be vigilant enough to diagnose and manage timely.
