Abstract
Method:
A retrospective study was performed in patients following their first aquaporin-4 (AQP4)-IgG or myelin oligodendrocyte glycoprotein (MOG)-IgG-ON. BCVA in ON eyes at 1 month and 3 months (BCVA1mo and BCVA3mo) was correlated with time to IVMP (days). All pts were followed for next three years for visual stability, recovery, recurrences and visual recovery after recurrent episode with same treatment protocol.
Results:
A total of 32 patients had ON (25 AQP4-seropositive neuromyelitis optica spectrum
disorder and 7 MOG-IgG-ON). Of the total patients, 9 presented later than 7 days from onset. ROC analysis revealed that the optimal criterion of delay in IVMP initiation was ≤3.5 days, with a sensitivity and specificity of 75.4% and 78.9%, respectively.
Conclusion:
Hyperacute treatment with IVMP increases the chance for good visual recovery (20/20 vision) and even a 7-day delay in IVMP t/t was detrimental to vision for the long-term visual recovery and outcome in this patients.
