Abstract
A 70 yr old female presented with swelling over right UL since 10 years with sudden increase in size over 4 months a/w pain and discharge. On examination, a large growth measuring 10 x 15 cm was seen arising from right UL with necrotic bleeding surface,involving the whole of upper eyelid with extension up to the eyebrow area. CT scan showed a lobulated lesion measuring 10 cm x 12 cm x 15 cm in pre septal area in right orbit with mild extension into bony orbit abutting globe and nasal bone medially. Surgical excision of mass with reconstruction of the upper lid with a right para-median forehead flap was done. The inner layer of UL was reconstructed with buccal mucosa graft. HPE revealed it to be sebaceous gland carcinoma. Post op results were satisfactory with good cosmetic outcome with no recurrence even after 6 months.CONCLUSION : Lid reconstruction using the para median forehead flap is an innovative technique to achieve good cosmesis in such patients with large tumors.
