Abstract

A 52 year old male presented with watering, redness and grittiness of right eye following splashing of diluted Dettol solution for ocular irritation. His vision was 6/9 OD, 6/6 OS with a 2×2 cm corneal epithelial defect OD. Eye was patched with antibiotic ointment after a thorough eye wash. On follow up the epithelial defect showed no signs of healing, increased in depth. Massive conjunctival chemosis and lid edema was noticed on day 3 of follow up. BCL application to hasten the corneal healing was not possible due to massive chemosis and required patching the eye which reduced the frequency of the topical medications. A worsening epithelial defect initially restricted us to use only NSAID’S for conjunctival chemosis. This case is unique for its mode of injury and the contradictory treatment of the two ocular signs, as management of one condition led to worsening of the other. Use of topical steroids with intermittent patching led to resolution of all signs and symptoms over 3-4 weeks.
