Abstract
A 5 year old child with left eye 45 PD esotropia underwent left eye MR recession and LR resection.The surgery was uneventful with no post-operative complications. 3 weeks later, the patient presented with a smooth pinkish-yellow sessile mass at the site of the lateral rectus muscle, in the left eye. The swelling was excised and patient was started on topical antibiotic and steroid eyedrops, but showed recurrence within 4 days. An aggressive treatment of topical fortified vancomycin eyedrops qid, gatifloxacin eyeointment od, dexamethasone eye drops 6 tid, injectable vancomycin for 10 days followed by oral linezolid for 5 days was given. At the end of one month there was a complete resolution of the swelling. This presentation of a swelling over the site of surgery with a definite recurrence even after excision and it’s resolution with aggressive topical steroids and systemic antibiotics support the diagnosis of a pyogenic granuloma – a comparatively rare and relentless complication.
