Abstract
Five children aged 6 to 16 years with exotropic type III DS were prospectively evaluated for up / down-shoots, extorsion, pattern strabismus, adduction/abduction limitations abnormal head posture (AHP), primary position exotropia (XT). Seven to 9 mm recession of DS eye LR with ¾ width full muscle inferior transposition was done. Results were analyzed at 12 weeks.Visual acuity was 6/9 or better in all. Face turn to opposite side in all, severe upshoots in (3),down shoots in (2), grade 1 -2 extorsion in(4) and grade -1 to -2 duction limitation of the affected left eye were noted. Post operatively over-shoots and extorsion were abolished, PP alignment improved by 23.60±7.80 prism diopters (PD), mean correction/mm recession was 1.78±0.31 PD ,torsion improved by 1.6±1.14, adduction limitation improved by 1.0±0.71. Abduction and V pattern did not worsen. Conventional recession with full muscle Inferior transposition of DS eye LR is effective in eliminating severe up/down-shoots and extorsion
