Abstract
Purpose: To report a rare case of IgG4-related disease masquerading as a metastatic malignant melanoma of the choroid. Methods: An 18-year-old female presented with features of right ocular inflammation and intraocular mass lesion. A complete ophthalmological evaluation of the patient was done along with ultrasonography, magnetic resonance imaging and positron emission tomography scan. Based on the clinical and radiological features, a diagnosis of malignant melanoma of the choroid with pulmonary metastasis was made. Results: The patient underwent enucleation and the tissue was sent for histopathological examination. Histopathology revealed features suggestive of IgG4-related disease. The patient’s serum IgG4 levels were found to be raised. She was started on systemic steroids. Conclusions: IgG4-related disease, albeit rare, should be considered in the differential diagnosis of a patient presenting with features of metastatic malignant melanoma of the eye.
