Abstract
A 65 year old male patient presented with swelling of left eye including periorbital area with severe pain, associated with diminision of vision with ulcerated area over left eye with bloody discharge.
The patient was a diagnosed case of pemphigus vulgaris since 2 years & was on steroid treatment, atrophic rhinitis & has chronic history of alcoholism, thus has immunocompromised status. On examination, left eye was swollen, with ulceration with visible multiple larvae and bloody discharge. Vision was pl+. Right eye was within normal limits. CT scan revealed soft tissue density leisions involving left orbit, nasal cavity and ethmoid air cells. 2days later, MRI revealed destruction of left eye ball.
Patient was given antibiotics and antiparasitic drugs.Turpentine oil packs were given & maggots removed, identified as larvae of Oestrus ovis by microbiology. Regular dressing were done & wound was healing secondarily. Patient is planned for evisceration of left orbit in subsequent month.
