Orbital myositis is an idiopathic inflammatory disorder of single or multiple extraocular muscles characterised by painful diplopia with response to oral steroids. Reporting a case of 46 year old male with swelling and drooping of right upper eyelid 5 months back, diagnosed as Ocular Myasthenia elsewhere and treated with neostigmine. He consulted a neurologist for the same due to lack of improvement.2 months later came with pain, redness, double vision and swelling of right eye. Vision was 6/6 parts in right eye and 6/6 in left eye.Right eye showed injection over insertion of lateral rectus muscle and pain on abduction. EOM was full. MRI orbit showed diffuse enlargement of lateral rectus muscle with irregular margins and involvement f tendinous insertion. He was started on oral and topical steroids . 2 weeks later showed improvement. steroids were tapered over next 4 weeks. although ocular myositis is a rare disease, prompt recognition, diagnosis and treatment gives good results.
