Abstract
A 37 year old female referred to neurologist for i.v steroids & neuroimaging with diagnosis of optic neuritis induced optic atrophy took second opinion with ophthalmologist before neurology consultation.Ocular&fundus examination of RE were within normal limits.BCVA of LE 6/9,NV:N8.RAPD present,red green color vision defect noted.Mild constriction of left visual field.LE showed significant pallor of NRR,vertical CD ratio 0.7:1.Humphrey visual field analyser 24-2 RE normal,LE showed significant superior altitudinal scotomas&inferior nasal step&inferior incomplete arcuate scotoma present also generalised depression of field noted.OCT of LE showed decreased RNFL thickness.MRI Brain(plain)small extra axial lesion in left basi frontal region indenting the optic chiasm on left side,CEMRI Brain-intense homogenously enhancing mass lesion in left suprasellar region involving optic nerve probably meningioma.VEP:P100 latency prolonged in LE.Patient was referred to neurosurgeon for further care.
