Abstract
Alkalinization of an anesthetic medication enhances the rate of onset of regional anesthesia. It hastened the effect of peribulbar anesthesia given prior to routine cataract surgery.In 240 cataract patients,the first eye (groupFE) received a peribulbar block with 6ml of 2% lignocaine with 15IU/ml of sodium hyaluronidase at a pH 4.6.After one week the second eye(groupSE) was operated under peribulbar block with 6ml lignocaine 2% mixed with 7.5% sodium bicarbonate at a pH 7.4.The time taken for onset of akinesia in both the situations was measured to last minute using stop watch.35%,60% and 5% of patients in groupFE had complete akinesia at the end of 5,7 and 10min respectively.68% in SE had complete akinesia in 5min and 100% in 7min.No anesthesia related ocular or systemic complications were seen in any group.
Sodium bicarbonate is faster than hyaluronidase with reduced time of onset of akinesia.Hence,it is cost effective and time saving where a large number of surgeries are performed.
