Abstract
Purpose: To evaluate the efficacy of inferior oblique anterior transpositoning (IOAT) with differential resection of muscle belly in cases of dissociated vertical deviation (DVD) with inferior oblique over action (IOOA).
Methods: 18 eyes of 9 patients with DVD of > 15 PD and IOOA > +3 were included. IOAT was done along with resection of IO muscle to augment its effect (4-7 mm resection of IO according to severity of DVD).
Result: Mean age of surgery was 8.2±3.7 years, Five had esotropia and 4 had exotropia with average deviations of 51.25±15.4 PD and 40±15.8 PD, respectively. Mean pattern corrected was from 27.22±6.6 PD to 5.33±3.4 PD (p<0.001). DVD was corrected in all cases from 18.22±3.2 PD to 1.11±2.2PD. Mean IOOA preoperative was 3.33±0.5 which corrected to 0.22±0.4 (p<0.001). One patient developed antielevation in which 7 mm IO resection was done. Conclusion: IOAT may be augmented safely by resecting the IO muscle in cases of DVD >15 PD with severe IOOA with favourable outcome.
