Abstract
A 41 yr old male patient referred to ophthalmology opd from neurology dept with compliants of weakness of both lower limbs since 10 days and sudden loss of vision BE since 2days. On examination visual acuity of RE is CF 1meter NIF, LE is 6/36 NIF. Slit lamp examination of BE were normal. Pupils- BE shows afferent pupillary defect . Fundus picture of BE was normal. Confrontation test shows BE-mild peripheral restriction of visual fields. Colour vision- desaturation of colours (RE> LE). MRI shows areas of altered signal intensity in left occipetal periventricular region; might be demyelination. There is csf distention of bilateral optic nerve sheaths and there is hypertrophy of ligamentum flavum noted at D9-D10, D10-D11. VEP shows prolonged p100 both eyes. Serum aqaporin4 antibody detected. Patient was diagnosed as neuromelitis optica and was kept on systemic steroids and immunosuppresants.Now there is improvement in symptoms & VA.He was maintained on oral steroids and immunosuppresants.
