Abstract
Upper eyelid retraction is a surgically challenging situation. Transconjunctival levator aponeurectomy is a newly described surgery for correction of lid retraction secondary to Graves’ orbitopathy. In this study, we implemented this surgical technique in 8 upper eyelids of patients older than 18 years old with moderate to severe lid retraction. In this procedure, levator aponeurectomy was performed via the transconjunctival approach. Upper marginal reflex distance (MRD1) was measured before the surgery and at 1 week, 1 month, and 3 months after the surgery. MRD1 was reduced significantly from preoperatively (mean: 7.24 mm) to 1 week after the surgery (mean: 3.92 mm) . 1month and 3 months after surgery, mean MRD1 was 4.29 mm and 4.82 mm, respectively, showing that lid retraction was improved significantly. Lateral levator aponeurectomy via the transconjunctival approach is a simple, easy, scar-less, safe and efficient procedure that has good stable outcome.
