Abstract
CASE REPORT:
A 25 Year old male patient presented with painless diminution of vision left eye since 15 days,later in right eye.Patient had hemicranial headache.vision right eye-cf-2mts,left eye-hmcf.left eye pupil had rapd+,fundus showed b/l papilledema.Blood investigations showed hyperhomocystenemia
Management: Patient was referred to neurologist,started on i.v,oral steroids,Iv mannitol,clexane,Tab Homin,acetazolamide.MR venogram showed superior sagittal sinus,cortical vein thrombosis
There was no improvement ,Patient underwent Thecoperitoneal shunt
Result: Vision improved to 6/36 in right eye and cf 2 meters in left eye post surgery
Conclusion:
Any young adult with B/L papilledema,blurring of vision should undergo Neurological examination and MR venogram,screened for homocysteine level as hyperhomocysteinemia is prothrombotic condition,shunt surgery improves the visual outcome
