Abstract
GHPC is a rare, idiopathic disorder primarily involving the choriocapillaris, RPE and photoreceptors. Disease is bilateral but asymmetric. Lesions first appear near the optic nerve head, extending centrifugally in a pseudopodial manner. Recurrences develop a geographic pattern of chorioretinal scarring. We present a case of a young male with diminution of vision in 1 eye for 2 weeks. Various lab investigations ruled out infective etiology and a clinical diagnosis of Serpiginous choroiditis was made. Examination suggested a prior, similar episode in the other eye. OCT revealed RPE elevations (active lesions) and IS-OS disruption and thinning (Healed lesions). The clinical course was documented on serial fundus photographs. This helped us titrate the dose of oral steroids and observe different stages of healing and activity of the disease over a period of 3 months. Treatment of initial or recurrent acute disease with corticosteroids yielded remission and preserved vision in both eyes.
