Abstract
Aim:To study effect of Valsalva on Intraocular Pressure(IOP) & Ocular Blood Flow(OBF) in Primary Open Angle Glaucoma(POAG) & Ocular Hypertension(OHT)
Methods:Hospital based, cross sectional, case-control study in 45 subjects. History/examination including IOP & OBF: End Diastolic Velocity (EDV), Peak Systolic Velocity(PSV) & Resistivity Index(RI) in three retro-bulbar arteries -Central Retinal Artery(CRA), Ophthalmic Artery(OA) & Short Posterior Ciliary Arteries(SPCA) were measured pre/post Valsalva
Results:A short term increase in IOP that was maximum in POAG(p=0.001). Decrease in PSV(10.43 to 8.18,p=0.0001) & EDV(p=0.001) was seen most significantly in SPCA with increased RI(p=0.023) post Valsalva in POAG. No significant difference was seen in between the groups in postValsalva OBF, although resting OBF was lower in POAG than OHT
Conclusion
The OBF decreased in all subjects post valsalva but as resting OBF values were less in POAG/OHT so decrease was more evident in these patients
