Abstract
A 13 year old female presented with symmetrical drooping of both upper lids and a prominent skin fold near medial canthus of both eyes since birth. It was constant and she has attained secondary sexual characters. It was not associated with any diminution of vision, trauma, diplopia or pain. She hasn’t attained menarche. Her father, 2 of her 4 siblings and nephew has similar complaint.
On ocular examination, she has a chin lift, increased forehead wrinkling and high arched brow. Her Vertical Fissure Height is 4.5mm, Negative MRD1 is 1mm, poor LPS action of 3mm, good Bell’s phenomenon, absent Marcus Gunn Jaw winking and lagophthalmos in both eyes. She has a symmetrical prominent epicanthal fold and palpebral fissure length of 17mm. She has a BCVA of 6/6, rest of the anterior segment and fundus is normal in both eyes. B/L V-Y Plasty followed by B/L Tarso-frontalis Sling Surgery a month later was done.
On post-op evaluation, VFH was 9mm, MRD1 was 3.5mm and PFL was 21mm in both eyes.
