Abstract

Pseudoexfoliation (PXF) is diagnosed by the appearance of grayish white flaky material on the anterior lens capsule and/or pupillary margin. Though of rare occurrence, PXF material deposition on the intraocular lens(IOL) has been reported, we are reporting a similar case. 69yrs old male, presented with diminished vision in right eye since 6months. Undergone cataract surgery 5yrs back to both eyes. Vision in right eye was 6/60, intra ocular pressure 40mmhg, open angles, pupil- relative afferent pupillary defect and PXF material deposits on anterior surface of IOL. Fundus-C:D ratio 0.9, advanced field defects. Left eye was normal. Importance of this case is the need of a thorough examination, even in pseudophakic eyes to look for PXF material on IOL, for it could be the only sign in the diagnosis of secondary open angle glaucoma and exfoliative material in pseudophakic eyes years after IOL implantation suggesting a long standing production in the absence of natural lens friction.
