Abstract

We present a case report of bilateral axial proptosis having the coexistence of thyroid orbitopathy(TAO) and orbital pseudotumor(OP). A forty-four-year female presented with swelling, pain, and protrusion of both eyes for one month. There was a past history of trauma in the right eye (RE)15 years ago. Her vision in RE was hand movements and 6/9 in the left eye(LE). There were lid edema, conjunctival congestion, chemosis with proptosis, and restricted extraocular movements in both eyes(BE). The anterior segment revealed old traumatic sequelae in RE; normal in the LE. RE fundus could not be viewed; LE normal fundus. B scan (BE) was found to have all extraocular muscle(EOM) enlargement. CT orbit showed diffuse enlargement of all the EOM belly and tendon insertions more of a typical pseudotumor. Thyroid profile found to have high Anti-TPO suggestive of Hashimoto’s thyroiditis. The patient was started on intravenous steroids. Improved symptomatically. Though rare, TAO and OP can coexist.
