Abstract
A previously healthy 9 year old girl presented with esotropia of left eye since 8 months with no significant past history. Her EOM was full with neurological examination , fundus and MRI brain were within normal limits. Hirschberg test had esotropia of in left 30° eye & PBCT demonstrated esotropia of 60 PD on fixing each eye in primary gaze .She was treated by occlusion therapy and underwent complete neurological evaluation. After 8 months with no response to occlusion therapy surgery was planned. She underwent left eye medial rectus recession of 8 mm & lateral rectus resection of 5mm under general anaesthesia. postoperatively her eyes were orthophoric. This report is to emphasize the importance of clinical diagnosis and systemic approach to a challenging case yielding good post operative outcome.
