Abstract
We report a case of unilateral proptosis in female of age 32years presented with complaints of diminution of vision unilateral headache towards right side,unilateral exophthalmos of right eye associated with chemosis and restriction of movements.Patient also complained of diplopia. patient had afferent pupillary defect.With unilateral proptosis differential diagnosis that often comes in mind are orbital pseudotumour,orbital cellulitis,cavernous sinus thrombosis,or intraorbital neoplasms.Radiological investigations like CT/MRI imaging and CT/MR angiography is necessary to rule out the differential diagnosis.Patient initially presented to ophthalmologist with concern of reduced vision and red eye with chemosis for further management.Hence in this patient there is when MR angiography was done it suggested of right internal carotid artery-cavernous sinus fistula.Cavernous sinus fistula can be traumatic or spontaneous,and delay in diagnosis may lead to poor visual prognosis.
