Abstract
Purpose: to innovate a new technique to remove hard cataract through a small incision. Method: hard cataract can be removed through a small incision (around 3mm) during small incision cataract surgery. You have to make a normal 3 mm straight incision 2mm from limbus, then make “INVERSE L” by adding one end of your main incision with another 3mm incision posteriorly. New “INVERSE L” incision will be well inside the astigmatig funnel or incisional tunnel. Then make tunnel as routine. While nucleus delivery you have to hold the junction of the L incision and nucleus will be delivered by a rotating movement. Result: with this method, surgically induced astigmatism will be very less compare to traditional 6-6.5mm incision. Post operative clear cornea will be extra beneficial. Conclusion: this novel “INVERSE L” INCISION technique during SICS for large hard nucleus, is easy to learn, reproducible with less post operative astigmatism, should be popular among ophthalmologists.
