Abstract
Background
Managing large angle exotropia (>75PD) at times can be laborious with the various techniques
available. This case series, presents true muscle transplantation, a simple novel technique to attain
satisfactory results with minimum hiccup.
Methods
7 patients (4 female, 3 male) of mean age 42+/-17.78 yrs with sensory large angle exotropia
(70-100PD) underwent detailed pre-op evaluation and refraction followed by muscle transplantation
surgery. The stump resected from MR was transplanted to LR with 5-0 nylon suture, along with LR
recession. Follow up was done upto 6 months.
Results
The mean deviation of 77.5+/-1.33PD was rendered orthophoric in 2 patients, reduced to 10PD
residual exotropia in 3 patients and <5PD in 2 patients. No patient had lateral incomittance and the
results were stable beyond 6 months.
Conclusion
True muscle transplantation for large angle exotropia gives satisfactory and stable results with
additional benefit of being a simple uniocular surgery.
